A chronic disease management care plan is suitable for eligible patients who have had a medical condition for six months or longer. These plans are designed to give patients a clear pathway for managing their condition with support from their GP, practice nurse, and allied health professionals where required.

For patients who need extra support, the clinic may also arrange a Team Care Arrangement. This allows the GP to coordinate treatment with other healthcare providers, including:
